Dermal filler is one of the most powerful tools in aesthetic medicine — and one of the most frequently misused. The difference between a result that looks natural and one that doesn’t comes down almost entirely to the physician’s philosophy, their knowledge of facial anatomy, and their willingness to say no when no is the right answer.

At Intentional Self Aesthetics, dermal filler treatments are performed by Dr. Teresa Alasio, MD. Her approach is grounded in a single principle: filler should restore what age has taken, not redesign what nature created. The goal is always a natural result — a face that looks rested, refreshed, and like itself, not heavier, rounder, or different.

If the right answer for your face on a given day is less filler than you came in asking for, Dr. Alasio will tell you that. And if she believes a treatment would cross the line from restoration into overcorrection, she will not do it. That level of clinical honesty is what physician-led care actually means.

How Dr. Alasio Approaches Dermal Filler

Filler is not a formula. What works beautifully on one face would look wrong on another, and the physician’s job is to understand the difference before picking up a syringe.

History first.

Before Dr. Alasio assesses anything visually, she wants to understand what matters most to you. What is your greatest concern? What have you noticed changing? Have you had filler before, and how did you feel about the results? These questions matter because treatment that doesn’t address what the patient actually sees is treatment that misses the point, no matter how technically correct the injection is.

Volume restoration, not facial redesign.

Dr. Alasio uses filler to replace what has been lost with age: volume in the cheeks, temples, and lips, structural support in areas where bone and fat have diminished, and softening of lines and folds that deepen as the underlying framework recedes. What she does not use filler for is reshaping a face into something different from what it naturally is, or filling beyond what the face’s anatomy can support. Filler is a restoration tool, not a construction tool.

What she looks at:

Dr. Alasio assesses skin quality, fat compartment loss, and bone structure in every filler patient. These three layers interact with each other and with filler differently. In the era of GLP-1 weight loss medications, facial fat loss is a particularly important consideration — patients who have lost significant weight through semaglutide or similar treatments often experience accelerated facial volume loss that requires a different and more careful approach than standard age-related volume replacement. Older patients may also show signs of bony remodeling that changes how and where filler is most appropriately placed.

The overdone conversation.

Many patients come to Dr. Alasio having seen the pillow faces and cartoonish lips that have become unfortunately associated with filler in popular culture, and they are understandably nervous. She addresses this directly in every filler consultation.

The conversation covers the line between undertreating and overtreating, how swelling affects the immediate appearance of filler and why results need time to settle before any additional volume should be considered, and why going slowly — treating conservatively and assessing over time — consistently produces better outcomes than trying to achieve everything in one session. Filler can always be added. Dissolving it is a process. Starting slow is always the right clinical choice.

If at any point Dr. Alasio believes that what a patient is asking for would produce a result that looks unnatural or inappropriate for their anatomy, she will say so clearly and decline to proceed. This is not a limitation of the practice. It is the standard of a physician who takes the outcome seriously.

Where Filler Works and How Dr. Alasio Thinks About Each Area

 

Cheeks and midface Volume loss in the midface is one of the earliest and most significant signs of facial aging. As the fat compartments in the cheeks diminish and descend, the face loses its youthful projection and the skin begins to fold inward and downward. Restoring cheek volume is one of the highest-impact filler treatments available — when done correctly, it lifts the entire midface, softens nasolabial folds indirectly, and restores a natural, three-dimensional contour. Dr. Alasio assesses the distribution of remaining fat and the degree of bony change before determining how much volume to restore and precisely where to place it.

Lips Lip filler is one of the most requested and most often overdone treatments in aesthetics. Dr. Alasio’s approach is to enhance the lips’ natural shape, restore volume that has been lost with age, and improve definition at the border — not to dramatically increase size or create a shape inconsistent with the patient’s anatomy. She uses hyaluronic acid filler specifically formulated for the lip tissue, and she treats conservatively, prioritizing softness and natural movement over volume. If your lips are naturally thin and your goal is a dramatic size increase, that is a conversation she will have honestly with you about what is achievable without compromising the natural appearance she insists on.

Nasolabial folds and marionette lines The folds that run from the nose to the corners of the mouth, and the lines that extend downward from the corners, are among the most commonly treated areas with filler. Dr. Alasio approaches these areas with the understanding that they are often better treated by addressing the volume loss in the midface above them than by placing filler directly in the fold itself. Sometimes direct treatment is appropriate; sometimes it isn’t. The right approach depends on the individual anatomy.

Jawline and chin Filler placed along the jawline and chin can sharpen definition, restore structural support lost with age and bony remodeling, and improve the overall proportions of the lower face. This is an area that rewards precise placement and conservative dosing, and where anatomical knowledge is particularly important given the proximity to important vascular structures.

Temples Temple hollowing is one of the most overlooked aspects of facial aging, and one of the most significant. When the temples deflate, the face takes on a skeletal, aged quality that no amount of midface filler can fully correct because the structural loss is occurring at the outer frame of the face. Dr. Alasio assesses temple volume at every filler consultation and addresses this area when appropriate.

Under eyes (tear trough) The hollowing that develops under the eyes with age creates shadow and the appearance of fatigue that is often the first thing patients say they want to address. This area requires significant anatomical precision and careful patient selection. Dr. Alasio does not place hyaluronic acid filler in the tear trough. The tissue in this area is exceptionally thin and the anatomy is complex, and she considers the risks associated with HA filler here significant enough to use PRF — Platelet Rich Fibrin — as her treatment of choice instead. PRF integrates naturally with the delicate under-eye tissue, stimulating gradual collagen production and improvement without the complication risks that come with HA filler placement in this location. This is a deliberate clinical decision, not a limitation.

Dermal Filler Care Instructions

To make sure you get the most out of your dermal filler treatment, follow these simple pre- and post-care steps. They can help minimize any side effects and keep your complexion looking fabulous.

Pre-Care Instructions

  • Avoid consuming alcohol and do not use Retinol-based products (including those with vitamin A and C) 3 days before your appointment.
  • Discontinue Omega-3 and Vitamin E supplements, NSAIDs, and anticoagulants 7 days prior.
  • Let us know if you have any viral skin conditions such as Herpes Simplex Virus.

Post-Care Instructions

  • Avoid blood thinners, NSAIDs, alcohol, excessive sun exposure, and physical exertion for 2 days.
  • Apply ice packs 15 to 30 minutes at a time to reduce discomfort.
  • Do not apply makeup for 24 hours.
  • Adhere to specific post-treatment instructions given during your appointment.
  • Avoid intense heat (including hot tubs, saunas, sunbathing, or tanning) for 48 hours after.

Juvederm is a smooth-consistency hyaluronic acid filler that integrates naturally with soft tissue and works particularly well in areas of high movement such as the lips and cheeks, where flexibility and a soft, natural feel matter most.

Restylane has a more cohesive formulation that provides excellent structural lift and definition, making it well-suited for areas like the jawline, chin, and tear trough where precise placement and structural support are priorities.

Both product lines include multiple formulations engineered for specific depths and treatment areas. Dr. Alasio selects the appropriate formulation for each individual treatment decision — not a one-size-fits-all product choice applied to every patient.

All hyaluronic acid fillers used at Intentional Self Aesthetics are FDA-approved, reversible with hyaluronidase if needed, and carry established safety profiles when placed by a trained physician.

Ultrasound-guided assessment with Clarius:

At Intentional Self Aesthetics, Dr. Alasio uses the Clarius handheld ultrasound device as part of her filler practice — a capability that is rare in aesthetic medicine and that reflects the clinical standard she holds herself to.

Ultrasound allows Dr. Alasio to visualize filler that is already present in the face before placing any new product. This is particularly valuable for patients who have been treated elsewhere and may not know exactly what was placed, where, or how much. Placing additional filler on top of unassessed prior treatment without understanding what’s already there is one of the primary causes of overcorrection and complication. Ultrasound removes the guesswork.

The Clarius device is also used to guide precise placement in areas where vascular anatomy makes accuracy critical, and to locate and assess filler when dissolution is clinically indicated.

Cellenis Derma PRF as an alternative to filler:

Not every patient is best served by hyaluronic acid filler, and Dr. Alasio makes this assessment individually. PRF — Platelet Rich Fibrin — is a regenerative injectable made from a small sample of the patient’s own blood, centrifuged to concentrate the platelets and growth factors, which are then re-injected to stimulate collagen production and tissue regeneration naturally.

Dr. Alasio tends to steer younger patients toward PRF over traditional filler. Younger patients typically have platelets in better biological condition, which means stronger biostimulator properties and a more robust regenerative response. For these patients, PRF achieves meaningful improvement in skin quality, fine lines, and early volume concerns through the body’s own healing mechanisms rather than through an external substance.

PRF is also Dr. Alasio’s treatment of choice for the under-eye area. She does not place hyaluronic acid filler in the tear trough. The tissue under the eye is thin, the anatomy is complex, and the risks associated with HA filler placement in this location are significant enough that she considers PRF the safer and clinically superior option. PRF integrates naturally with the delicate tissue in this area and stimulates gradual, natural-looking improvement without the complications that can arise from HA filler near the eye.
