You Don’t Have to Guess What’s Under Your Skin

I get some version of this call almost every week. “I don’t know what filler I have, or how much, or where it actually is.” “I had filler somewhere else years ago and now there’s a lump.” “My lips feel uneven and nobody can tell me why.” “I want it gone, but I’m scared to have someone dissolve something they can’t see.”

I understand the hesitation. Most providers place filler and dissolve filler by feel and by memory of anatomy. That works most of the time. But when the history is unclear, or the result isn’t what you expected, guessing is no longer good enough.

I trained as a pathologist before I became an aesthetic physician. My entire career has been built on looking closely before I act. Diagnostic ultrasound lets me bring that same discipline to filler. I can see the product itself, exactly where it sits, how deep it is, and how it relates to the vessels and structures around it. Then I treat what I actually see, not what I assume is there.

What is Ultrasound Filler Detection?

Ultrasound filler detection uses a handheld diagnostic ultrasound probe to image the tissue under your skin in real time. Filler shows up clearly on ultrasound, whether it was placed last month or ten years ago, and whether or not you remember what it was.

During your visit, I scan the treatment area and identify:

  • Whether filler is present at all, and how much
  • The depth of the product (superficial, mid-dermal, or deep to muscle)
  • Whether it’s sitting where it should be, or has migrated
  • Nodules, lumps, or areas of firmness, and whether they are filler, scar tissue, or something else
  • The location of major blood vessels relative to the filler, before any needle goes in

This is diagnostic information you simply cannot get from a consultation and a fingertip alone.

Who This Is For

  • You’ve had filler from another provider and don’t have clear records of what or how much
  • You’re feeling a lump, bump, or asymmetry and want to know what it is before deciding what to do
  • You had filler placed elsewhere or abroad and want a professional assessment before your next treatment
  • You’re experiencing prolonged swelling, firmness, or a change in appearance months after treatment
  • You want your chin, jawline, lips, tear troughs, or cheeks fully dissolved and want the confidence of a guided, visualized removal
  • You’re considering new filler in an area that’s already been treated and want to know exactly what’s there first

Ultrasound-Guided Filler Dissolving

Once I can see the filler, I can treat it precisely. For hyaluronic acid fillers, I use hyaluronidase, an enzyme that breaks the filler down, injected directly into the product under real-time ultrasound guidance.

This changes what dissolving actually means. Instead of injecting broadly across an area and hoping to reach the filler, I watch the product dissolve in the moment, confirm how much has broken down, and adjust the treatment in real time. For deep or long-standing filler, or filler in a delicate area like the tear troughs or lips, this precision matters.

I performed this exact procedure recently for a patient with chin filler she wanted fully resolved. Ultrasound let me confirm the product’s location and depth beforehand, treat it directly, and verify the result before she left my office.

Why This Matters

Filler that migrates, sits too superficially, or accumulates over repeated treatments can cause visible lumps, unnatural contour, or a “puffy” look that no amount of skincare will fix. Some patients have layers of filler from different providers over the years and no idea how much product is actually in their face.

Ultrasound removes the guesswork on both ends. It tells us what’s really there, and it lets us treat it directly instead of broadly. For anyone who has felt unsure about filler they can’t explain, or nervous about dissolving something a provider can’t see, this is the more precise, more honest way to do it.
